Meals on Wheels of Greenville is pleased to announce their partnership with the Memory Health Program of Prisma Health to provide a new program in support of caregivers of dementia clients. The program, called REACH, stands for Resources Enhancing Alzheimer Caregiver Health and focuses on sessions that provide dementia caregivers the tools and strategies necessary to enable them to give the best possible care to their loved one while maintaining their own health and well-being.
With this new partnership, we’ve compiled a FAQ for your reference.
Who can benefit from this type of program? The REACH program is offered to the clients of Meals on Wheels of Greenville and their caregivers. The program is no cost and our Dementia Care Coach will help determine our clients’ eligibility. The REACH program delivers proven results and been known to improve healthy eating habits by 61%, decrease depression by 38%, and decrease burden in caregiver by 40%, all while positively impacting those being cared for.
Is there a cost for this program? The REACH program is free to all caregivers of Alzheimer’s and dementia clients of Meals on Wheels of Greenville. Prisma Health Center for Success in Aging was originally awarded the grant by the Administration for Community Living, which is within the U.S. Department of Health and Human Services. Prisma Health then awarded the funds to two agencies in the upstate, including Meals on Wheels of Greenville. Due to the generosity of Prisma Health, Meals on Wheels of Greenville is proud to have a full-time Dementia Care Coach on staff to assist with the REACH program and to ultimately further support our clients and their caregivers.
How does the program work? Our full-time Dementia Care Coach actively reaches out to all Alzheimer’s and dementia clients’ caregivers to determine eligibility and to begin the enrollment process. The REACH program includes twelve sessions per caregiver over a six-month period, which is customized to each caregiver and can be face-to-face or over the phone, depending on caregiver preference. The program is designed to be flexible for each caregiver and their loved one, ensuring a fully-customized experience to fit our client’s and their caregivers’ needs.
What if I know someone that is a caregiver of an Alzheimer’s or dementia patient? If you know someone that may benefit from the REACH program, please give us a call at 864.233.6565 to speak with our Dementia Care Coach. While the program is reserved for clients of Meals on Wheels of Greenville, we’d be happy to discuss whether the individual is eligible to receive meals and become a client, therefore making them eligible for the REACH program.
